Fourth Street Family Dental offers families and individuals with friendly dental care in Hermiston, OR. We are proud to serve patients of all ages with a comprehensive range of dental services. Our goal is to provide each patient with the best service in a comfortable and relaxed atmosphere. We focus on providing individualized care that helps patients achieve and maintain great dental health for life. Our staff is dedicated to providing the highest level of personalized, gentle and efficient dental care.

Appointments

We see patients by appointment only. We are currently accepting new patients. Please call our practice at 541-567-1693 to schedule an appointment. Once you make an appointment, that time will be reserved for you. If, for some reason, you are unable to keep your scheduled appointment time, please call us with 24 hour notice to reschedule and avoid any missed appointment fees. This also allows us to open up the appointment time to other patients.

What to Bring to your First Appointment

To provide you with prompt dental treatment, please be sure to bring the following items when you visit our practice for your initial visit:

  Insurance Information / Card

  Photo Identification

  Any medical records, including all medications and supplements

Financial & Insurance Information

To reduce the costs to our patients, while maintaining the highest level of professional care, we offer patients a number of ways to pay for treatment. Payment is due at the time of service. We accept Visa, Discover, or MasterCard. Fourth Street Family Dental offers our patients opportunities to finance their treatment through CareCredit (OAC).

Insurance Information

Our office team electronically submits all insurance claims and works with insurance companies to maximize patient benefits and expedite processing.

We provide professional dental care to you, our patient family, not to an insurance company. We will be happy to help you understand your policy. After you receive treatment, we will send you a monthly statement. Most insurance companies respond within 4 to 6 weeks. If your statement does not reflect your insurance payment, please contact our office. After your insurance has paid, any remaining balance is your responsibility.

You are fully responsible for all fees charged by this office regardless of your insurance coverage.

Consultations

If you are looking for a second opinion on dental treatments diagnosed for you, we are happy to help. Making sure our patients receive the course of treatment they need and deserve is important to us. We make every effort to educate our patients and help them understand the treatments we provide.

If you are referred to our practice from another office or are looking for second-opinion, you are required to undergo a complete examination by our dentist so you can be accurately diagnosed. Before your appointment with us, please ensure all of your available chart notes and x-rays are transferred to our office.

Please note, many offices require 7 – 14 business days to transfer records. This information, along with your examination allow us to give you a comprehensive treatment estimate. It is imperative that we have all of your available dental and insurance information in hand by the time of your appointment to ensure we give you an accurate treatment estimate.
